UB Neurosurgery’s work with subdural hematoma patients was recently highlighted on the cover of Operative Neurosurgery, a monthly peer-reviewed medical journal issued by the Congress of Neurological Surgeons.
The July 2026 issue featured research authored by UBNS Neurosurgeon Dr. Rosalind Lai, titled The Utility of Ultrasound Through a Sonolucent Implant in the Perioperative Management of Subdural Hematoma: Three Exemplificative Cases. The study examined the use of sonolucent cranial implants for ultrasound imaging in post-operative subdural hematoma patients.
Dr. Lai co-authored the study with UBNS team members Dr. Elad Levy and Dr. Jason Davies, as well as Ryan Hess and Eisa Hashmi. Their research was conducted between January 2024 and January 2025.
The study explored alternatives to traditional cranial imaging for subdural hematoma patients. Cranial imaging is crucial for post-operative monitoring, but is usually done via CT scans. While effective, these scans expose patients to radiation and require time, travel, and hospital resources.
Dr. Lai and her team researched the use of a special transparent cranial implant that is placed during surgery and allows ultrasound waves to pass through the skull. The implants used were ClearFit burr-hole covers from Longeviti Neuro Solutions.
“With this implant, we can use a handheld ultrasound probe at the bedside to look for fluid or blood re-accumulation under the skull," Dr. Lai said. "This can potentially change care in the future by using serial ultrasound to monitor recurrence instead of CT scans.”
The study highlights three cases involving patients who received and were given ultrasounds through the implant. They found the use of the ClearFit burr-hole cover during craniotomy was safe and practical, and that the quality of the images provided sufficient information to help patient management without requiring computed tomography.
“Our goal [with this research] is to reduce unnecessary imaging by using ultrasound to monitor for subdural hematoma recurrence,” Dr Lai said. “Potentially reducing radiation exposure while maintaining safe follow-up.”
